KapillarblutHistamine and DAO Enzyme Analysis – Biomarker Measurement
Activity of the enzyme diamine oxidase in serum.

View testBloating, pressure in the upper abdomen, diarrhoea, headaches, and in children restlessness and trouble concentrating. If symptoms keep coming after certain meals, it is worth looking at histamine, lactose, fructose and gluten.
You take the sample at home. A partner laboratory evaluates it. You discuss the lab report with a doctor or a non-medical practitioner.

Which one fits depends on what you have noticed. The guide below matches observations to methods.

Wind and a bloated stomach
Pressure or cramps after eating
Diarrhoea or changing stools
Nausea
Headaches through to migraine
Grogginess after meals
Concentration drops off
Sleep gets more restless
Redness on the face and neck
Itching
Hives
Known skin problems getting worse
Tummy ache with no clear trigger
Irritability in the afternoon
Restlessness at school
poor sleep
These symptoms can have many causes. A test shows values, not a diagnosis. A doctor or a non-medical practitioner puts them in context.
Histamine is a messenger substance the body makes itself, and it is also in many foods, above all matured and fermented ones. It is broken down mainly in the gut by the enzyme diamine oxidase, DAO for short.
Histamine reaches the gut through matured and fermented foods: mature cheese, red wine, sauerkraut, smoked fish, salami.
In the gut lining, the enzyme diamine oxidase breaks histamine down before it reaches the bloodstream. A second enzyme, HNMT, works inside the cells.
If the breakdown capacity is not enough, the histamine level rises. It is a matter of quantity, not a yes-or-no question: the same person tolerates more on one day than on another.
Histamine acts on blood vessels, the digestive tract and the nervous system. That is why flushing, headache, diarrhoea and palpitations appear in the same picture.
Low DAO activity, whether inherited or temporary
An irritated gut lining, where DAO is produced
Alcohol, which slows the breakdown and contains histamine itself
Certain medicines that block DAO
Large amounts at once: several histamine-rich dishes in one meal
DAO activity in serum
Histamine in stool
Histamine in blood
Zonulin and alpha-1 antitrypsin for the gut barrier
Also: vitamin B6, copper and zinc, which take part in the breakdown
There is no single lab value that proves histamine intolerance, and no agreed cut-offs. The values are one building block alongside a symptom diary and a dietary history. The condition itself is also judged differently across the specialist literature.
Four different mechanisms, four different methods. The symptoms resemble each other; the causes do not.
| Thema | What lies behind it | Typical reaction time | Verfahren |
|---|---|---|---|
| Histamine | Breakdown by the enzyme DAO is not enough for the amount taken in | 20 minutes to several hours | DAO in serum, histamine in stool or blood |
| Laktose | The enzyme lactase does not fully split milk sugar | 30 minutes to 2 hours | H2 breath test, plus a genetic test |
| Fruktose | The small intestine absorbs only a limited amount of fructose | one to several hours | H2-Atemgastest |
| Gluten / coeliac | Autoimmune reaction to gluten, damaging the small intestine lining | gradual, often over months | Transglutaminase antibodies in blood, work-up by a doctor |
| IgG to foods | Antibodies against food components; the significance is disputed among experts | not clearly assignable | IgG measured from blood, up to 270 foods |
Reaction times are rough guides, not fixed rules. Coeliac disease is an autoimmune condition, not an intolerance; a suspected case belongs with a doctor, and antibody testing only means something while gluten is still being eaten.
Children rarely describe stomach trouble precisely. Instead of “I feel unwell after eating”, it shows as restlessness, irritability, poor sleep or slipping concentration at school. The link to food is then easy to miss.
Because children and parents share the same household and, in part, the same gut flora, symptoms often turn up in several people. A joint test puts the values side by side.
Children say "I feel funny", not "I have bloating".
Restlessness, irritability and tiredness instead of clear tummy trouble.
Stool and urine are straightforward, breath testing from about age six.
Analyses in children only where there is a specific observation.
A guide, not a recommendation. If you are unsure, sort it out in the free initial consultation.
After milky coffee or yoghurt the stomach swells up
Symptoms usually come within two hours and depend on the amount.
Laktose-AtemgastestFruit, dried fruit or soft drinks cause trouble
The small intestine absorbs fructose only to a limited degree; the rest ferments in the large bowel.
Fruktose-AtemgastestRed wine, cheese or salami lead to flushing and headache
A typical histamine pattern: several systems react at once.

Here a broader look at gut flora and gut barrier helps more than a single test.

What a lab report means for you comes from a doctor or a non-medical practitioner, not from this page.
These tests do not replace a medical examination. With acute symptoms, contact a doctor. Do not cut foods out permanently on your own — a needlessly restricted diet can itself lead to deficiencies, especially in children.
With an allergy, the immune system reacts to a protein in the food, usually within minutes to a few hours. With an intolerance, an enzyme is missing or weak, or a substance is poorly absorbed. The reaction is slower, often only after hours, and depends on the amount. That is why many people tolerate small portions without trouble.
By the repetition. Keep a simple diary for two weeks: what, when, how much, and which symptoms followed and when. If a pattern emerges, that is the best starting point for an analysis and for the conversation.
The usual options are DAO activity in blood, histamine in stool and histamine in blood. Each of these values has its limits: there is no single conclusive test and no agreed cut-off values. The values are looked at together with what you have noticed, not on their own.
Because it measures directly what happens in the gut. You drink a defined amount of lactose or fructose. If the sugar is not absorbed in the small intestine, bacteria in the large intestine break it down, producing hydrogen that is breathed out through the lungs. If the breath value rises clearly above the fasting value, that points to an intolerance.
Yes. You get the test solution, the tubes and a timetable. You breathe into the first tube while fasting, drink the solution and repeat at fixed intervals over about two to three hours. The tubes then go to the laboratory together.
It measures IgG antibodies against food components. Whether raised IgG values cause symptoms is disputed among specialists: several allergy societies consider the test unsuitable for diagnosing an intolerance, because IgG is also found in people with no symptoms. In functional practice it is nonetheless used as a pointer for a temporary elimination phase. You should know this before you order.
The gut barrier has a say in what passes from the gut into the body. Markers such as zonulin and I-FABP are used to gauge barrier function. Histamine is also broken down largely in the gut lining. That is why, with persistent symptoms, both are often looked at: intolerance and gut status.
Yes, and it decides how meaningful the result is. For breath tests: fast for at least twelve hours, and avoid gassy or high-fibre foods the day before. If coeliac disease is suspected, gluten must not be left out beforehand, or the test comes back falsely negative. Certain medicines affect the DAO measurement. The exact notes are included with every kit.
Stool and urine samples are straightforward from toddler age. Breath tests require the child to breathe out into a tube in a controlled way, which usually works from about six. A fingertip blood sample is possible from four, but should be talked through with the child. If in doubt, settle it beforehand in the initial consultation.
No, not on your own and not permanently. An unnecessarily restricted diet can itself lead to deficiencies, especially in children. A value that stands out is a reason for a conversation, in which the elimination phase, its length and reintroduction are planned.
The result is ready 7 to 21 days after the sample arrives at the laboratory. The lab report is then available in the Evi app. Evi stores and displays; a human does the interpretation.
That happens, and it is useful information. Complaints after eating can also come from the amount, the speed, the make-up of a meal, from stress or from the gut flora. Gut analyses or a different angle are then the sensible next step.
