Men go for check-ups later, and often only once something hurts. The values that would give early warning start changing long before that — and they can be measured at home.

Four groups of causes work together. Three can be influenced, one only partly — and a single one of them costs nothing but an appointment.

Testosterone falls with age, and that decline is closely tied to weight and inflammatory conditions. So it can be influenced indirectly.

No PSA as a self-test — interpreting it needs an examination and a conversation.

No substitute for measuring blood pressure. It is the cheapest and one of the most important values there is.

A slow decline, faster alongside other illnesses

The age-related decline is well described and slow. What speeds it up is usually not age itself but what sits alongside it — weight, inflammation, metabolism.

If you measure, do it like this
Sample in the morning between 7 and 11 am, fasting — the value varies a lot over the day.
Confirmation by a second measurement on another morning.
No screening without symptoms: a value with no matching symptoms leads nowhere.

When should testosterone be measured?

In the morning between 7 and 11 o'clock, fasting — the value varies considerably over the day. A conspicuous result is confirmed on a second morning.

Without matching symptoms, a value has no consequence. We do not offer SHBG, or the free testosterone calculated from it, as a self-test; a medical practice can measure them — ask our expert network about it.

How much does testosterone fall with age?

By about 0.4 to 1 per cent a year from the age of 40. Where other conditions are present, the decline is considerably steeper.

In the DETECT survey, 19.3 per cent of the men measured in general practice had a low value — most often alongside excess weight and metabolic syndrome. Symptomatic hypogonadism, meaning symptoms and a low value together, was found in the European Male Ageing Study in 2.1 per cent of those aged 40 to 79.

Do you offer a PSA test?

No, and deliberately not as a self-test. A PSA value is hard to place without a physical examination, a history and a conversation — a raised value has many harmless causes.

Urological prevention is a statutory benefit from age 45. That is where PSA belongs.

Does this replace the GP check-up?

No. From 35 the check-up is a statutory benefit every three years and includes things a self-test cannot do — above all blood pressure and the physical examination.

Where do I start if I have never had anything measured?
